3. How can I schedule a WIC appointment?

4. What proofs do I need to bring to a WIC appointment?

For certification appointments, bring the following information with you. All 3 of these items are required to receive benefits. 

• Proof of income
• Proof of residence
• Your ID

5. What is the contract brand of formula used by WIC?

Missouri WIC has contracted with the Similac® brand to provide infant formula to recipients.

6. Can I transfer to the Joplin Health Department's WIC agency?

If transferring from another WIC agency in state or out-of-state, make an appointment with Joplin WIC by calling 417-623-1928, choose option 3. If no-one answers, please leave a message. You will be contacted to set an appointment date and time. At that appointment, you will need to bring the contact number for your previous WIC agency.
