Charges for Services
Clinical Service Fees
The Joplin City Health Department provides many free services to the general public. Some nominal fees are charged for certain services. Fees effective July 1, 2019. Patients are requested to pay according to the schedules below.
Child and Adolescent Vaccinations
Vaccines available include those recommended by the Advisory Committee on Immunization Practices (ACIP).
Vaccinations are provided through three program options.
1. The Vaccines For Children (VFC) Program provides vaccines for children age 18 and under who meet one of the following criteria:
2. The second program option is for vaccinations covered by insurance. Fees for these vaccinations are
dependent upon insurance coverages and will be processed through a program called Vaxcare.
3. The third option is for cash payment for the cost of immunization services (cost of vaccine and
administration) for those individuals who have insurance not covered by the Vaxcare program. Written
documentation will be provided for the individual to file with own insurance.
PLEASE CALL AHEAD AT 623-6122 TO DETERMINE WHICH OPTION WILL APPLY AND IF YOUR INSURANCE IS COVERED IN VAXCARE PROGRAM AND TO MAKE AN APPOINTMENT. CHILD MUST BE ACCOMPANIED BY A PARENT/GUARDIAN OR AUTHORIZED RESPONSIBLE ADULT. PLEASE BRING IMMUNIZATION RECORD TO APPOINTMENT.

Adult and Travel Vaccinations
Adult vaccinations are provided through two program options.
1. The first program option is for vaccinations covered by insurance. Fees for these vaccinations are
dependent upon insurance coverages and will be processed through a program called Vaxcare.
2. The second option is for cash payment for the cost of immunization services (cost of vaccine and
administration) for those individuals who do not have insurance or have insurance not covered by the
Vaxcare program. Written documentation will be provided for the individual to file with own insurance.

Clinical Testing Service Fee
Nominal testing/treatment fees are charged for certain tests available through the department.
Fees for Clinical Testing Services
Through joint arrangement with APO Inc. free HIV and Hepatitis C testing and case management services are provided regularly, when certain criteria is met, at the Joplin City Health Department. Call 624-5788 for information.
*Free STD tests for syphilis, gonorrhea, and chlamydia are also provided regularly at the Joplin City Health Department by APO Inc. Call 624-5788 for scheduling.
**Child lead testing and case management is conducted through an arrangement with the Jasper County Health Department. Call the Jasper County Health Department at 358-3111 to schedule a test at the Joplin City Health Department.

- American Indian or Alaskan native
- Underinsured (have insurance that does not cover vaccines or that camps coverage for vaccines, after the maximum coverage has been reached)

dependent upon insurance coverages and will be processed through a program called Vaxcare.
3. The third option is for cash payment for the cost of immunization services (cost of vaccine and

- Hib (Haemophillis influenza Type B--bacterial meningitis)
- Human Papillomavirus
- Measles, Mumps, Rubella
- Meningococcal (meningitis & septicemia)
- Polio
- Seasonal Influenza
- Tetanus Diphtheria
- Tetanus Diphtheria and Pertussis
- Varicella (chicken pox)
- All adult vaccines as listed above
