Community Health Needs Assessment (CHNA)

The 2019 Community Health Needs Assessment provides detailed local community health information and data and other public health issues. Every three years, a community health needs assessment is conducted in conjunction with the Ozarks Health Commission (OHC). This effort involves many stakeholders and partnering organizations including local health care providers and neighboring health departments.


Community Health Improvement Plan (CHIP)

The 2019 Community Health Improvement Plan was developed after the CHNA finalization by each entity involved in the CHNA process. This particular CHIP is a two-county focused plan (Jasper and Newton County). Overall, CHIP documents serve to guide local initiatives to focus on potential areas of improvement.
